Generations  – Hospice: Program offers palliative care and support to those who are terminally ill and their families. Nurses, social workers, chaplains, bereavement counselors, and volunteers are all part of the palliative care team. To be eligible for hospice services patients, families, and/or guardians must have the desire to receive palliative care. The programs team will conduct an initial evaluation to confirm eligibility.

Ascension at Home/Compassus: Companionship and support offered for those who have a life-limiting condition and their family. Team includes physicians, skilled nurses, social workers, hospice aids, spiritual care counselors, bereavement counselors, respite and caregiver support, and volunteers.

St. Croix Hospice:  Offers end-of-life care and advocacy for patient, family members and caregivers. Patient care services can be received wherever those with a limited life expectancy call home. A plan of care is developed by the patient, the patient’s family, and the primary physician. The plan is reviewed every 15 days to ensure that the plan is appropriate.
