ADRC of Marquette County Transportation is a service for the elderly (60 and over), veterans, and/or disabled persons who are unable to drive or have no family member to take them to medical appointments.  The Transportation Specialist will be available by phone to receive transportation requests Monday thru Friday from 8:00 AM to 4:30 PM (excluding holidays and special training days).  Transportation requests must be made at least 48 hours in advance (excluding weekends and holidays).  It takes time to arrange a ride and if you do not give enough notice, you may not be able to obtain a ride. There is a fee-for-service for each ride.

You can get free rides if you are enrolled in one of the following programs: BadgerCare Plus, Wisconsin Medicaid (including IRIS), Family Planning Only Services, Express Enrollment for BadgerCare Plus, Tuberculosis-Related Services Only Benefit.  Ride Options form for Medical Appointments

Call 866-907-1493 or 711 (TTY). Make sure you have:

    • Your name, date of birth, home address, and phone number
    • Your ForwardHealth member ID (the 10 numbers listed on your ForwardHealth ID card)
    • Your phone number and the street address where you want to be picked up
    • The name, phone number, address, and zip code of the health care provider you are seeing
    • The date and start time of your appointment
    • The end time of your appointment if you know it
    • Any special ride needs, including if you are bringing your child or a medically necessary escort or attendant
    • The general reason for the appointment (checkup, eye appointment, etc.)

Online: Go to Veyo’s Member Portal .  You will need to schedule rides at least two full business days before your appointment.
You will need to create an account. You will need your name, ForwardHealth ID, date of birth, and email address.
Rides can only be scheduled online for appointment locations you have already had a ride to with Veyo. If you want to schedule a ride to a new appointment location, please call 866-907-1493 (voice) or 711 (TTY).

All Vet’s Transport: Free transportation is available for Veterans to medical appointments at the VA hospitals in Madison, Milwaukee, and the out-patient clinics in Baraboo and Beaver Dam. Hours are restricted to appointments between 9am and 2 pm.  Vans do not have lifts, patients must be able to transfer from wheelchairs to vans independently.  No emergency/ Ambulance calls available.

Fox River Industries: Fox River Industries provides daily transportation for elderly and disabled employees in the Green Lake County area with Human Service vehicles provided through Disabilities Services, Inc. These vehicles are also made available for community use at competitive rates for those interested in providing transportation services to elderly and disabled residents of Green Lake County. Green Lake County:  All areas of Green Lake County are provided with “response to call-in”, door-to-door transportation services to any person 55 years of age and older or any handicapped person. The priority areas of the programs are to provide transportation to medical appointments, congregate meal sites, and other personal appointments necessary for daily living. Monday – Friday. Local rides need to be arranged 2 days prior to appointments, and 1 week for appointments in larger cities.

Vanpool: The State Vanpool program provides alternate transportation for state and non-state employees commuting to Madison from outside communities. Participants can join a group that is already established or if there are enough interested people, they can form a new Vanpool.
