Donated Dental Program: Wisconsin Dental Association program designed to bring dental care to Wisconsin residents who are unable to afford treatment because of a limited income that is clearly linked to a permanent disability, chronic illness or advanced age (65 and older). Program is by application only. Eligibility is determined based on a completed written application and phone interview by the DDS referral coordinator.
